ALTERNATIVE OUTLINE FORMAT OF THE NEXT STEPS:

  1. Transfer Meeting with client, case manager, and personal agent (PA)

    1. PA explains Full Access Brokerage and universal access

    2. Fill out initial paper work

      1. Release of information

      2. Acumen paper work

  1. Second meeting most likely is a home visit with client, family, providers and personal agent

    1. Start filling out the customer survey, identifying goals, dreams, likes, dislikes

    2. Spend time with client and family and get to know each other a little better

  1. Continue meeting if necessary to fill out the customer survey and develop relationship

    1. The personal agent will start looking for providers at this time

    2. The personal agent will start looking for potential resources at this

  1. The PA arranges a meeting for a Person Centered Plan with the client and the people they choose including family, providers, friends

    1. The personal agent will act as the facilitator for the meeting

    2. Map out the clients goals, likes, dislikes, and different areas of their life

    3. Arrange for any necessary steps to be taken

  1. Write the Individual Support Plan (also known as the ISP)

    1. Budget all the financial information

    2. Set up any providers for the client

    3. Set up any employment options for the client if they are interested

    4. Review the plan and check in with client on a regular basis

    5. Review the plan on the anniversary date, provide 90 day reviews and revisit customer survey

      1. identify any new goals

      2. implement any changes that need to take place
